The Growing Obesity Epidemic and Its Health and Financial Consequences in China

Over the past three decades, China has enjoyed impressive economic development. Chinese People have experienced many dramatic changes in their lifestyles thanks to the increases in family income and availability of food as a result of China’s economic reform and the growing global trade. People’s lifestyles are becoming increasingly sedentary. Compared to other groups, children and adolescents are likely to be affected to a greater extent by these changes, partially due to China’s ‘one-child policy'.
Increasing research including ours suggests that the prevalence of obesity has increased in China both in children and adults. With the increase in obesity, obesity- and diet-related chronic diseases such as hypertension, cerebrovascular disease (CVD), and type 2 diabetes also increased over the past decade and have became the most important preventable cause of death. On the other hand, data collected from pre-school children show that undernutrition remains a public health concern, particularly in poor and rural areas. This article discussed the obesity related issues in China, predominately based on some of our recent research.

Increasing Epidemic of Sexually Transmitted Diseases in China

Sexually Transmitted Diseases (STDs) were virtually eliminated in China in 1964 after Chinese government illegalized commercial sex and implemented active prevention and treatment programs among former sex workers (Chen et al., 2000). However, in the last three decades, the centralized economy in China made a transition to a more market-oriented economy, which brought significant changes in culture and social norms and dramatically influenced sexual attitude and behaviors among Chinese adults. One consequence of those changes is the significantly increasing prevalence of STDs in China(Abrams, 2001). The common STDs in China include gonorrhea, syphilis, AIDS, cervicitis, genital herpes, chancroid, lymphogranuloma venereum, and chlamydial infection.

Dr. Gordon Liu is a professor of economics in Guanghua School of Management, Peking University; and Director of the PKU China Center for Health Economic Research (CHER). He was previously a tenured faculty at the University of North Carolina – Chapel Hill (2000 – 2006); and University of Southern California (1994 – 2000). Dr. Liu serves on the Chinese Ministry of Health Expert Commission for Emergent Public Health Events and Chair of the Asian Consortium for the International Society for Pharmacoeconomics and Outcomes Research (ISPOR). He was the elected President (2004-2005) of the Chinese Economists Society. For professional services, Dr. Liu serves as Co-Editor of the ISPOR official journal, Value in Health, and Editor-in-Chief of the China Journal of Pharmaceutical Economics. His primary research interests include health economics, health capital investment and economic growth, and pharmaceutical economics.

Refocusing China’s Family Planning Commission in the 21st Century

China’s entry into an aging society calls for a transition of its population policy. The State Family Planning Commission (SFPC) could concentrate more on many essential health and human services for the next few decades. Prevention of birth defects and reduction of unnecessary cesarean births are cost-effective interventions that would enhance the health of the upcoming generations. Research and evaluation of modern contraceptive technologies are needed to promote reproductive health and help both the SFPC and users make well-informed decisions. Human Immunodeficiency Virus (HIV) and Sexually Transmitted Diseases (STD) testing and counseling deserve the joint efforts of the government, health services, and the society at large. Further, the SFPC’s collaboration with the Ministry of Health, Ministry of Agriculture and the National Women’s Federation can help to reinforce its role in the treatment of infertility and maternal diseases, the provision of public services for low-income families, and the prevention of domestic violence. The SFPC can also play a role in China’s increasing foreign aid, particularly in those countries where artificial contraception and HIV/STD prevention are needed.

Dr. Jeffery P. Koplan was the Director of the U.S. Centers for Disease Control and Prevention (CDC) from 1998 to 2002, and is currently the Vice President for Global Health at Emory University and the Director of Emory Global Health Institute. Dr. Koplan began his public health career in the early 1970s, and has worked on virtually every major public health issue, including infectious diseases, environmental health issues, chronic diseases and the health toll of tobacco, both in the United States and around the globe. Recently, Emory Global Health Institute received a $14 million, five-year grant from the Bill & Melinda Gates Foundation and established the China Tobacco Partnership program. Dr. Koplan is the principal investigator of the grant and is leading the partnership, which is devoted to reduce the burden of tobacco use in China.
In this interview with Drs. Zheng Li and Feijun Luo, Dr. Koplan talked about the Tobacco Partnership program, his over 30-year of involvement in public health in China, his observation on the changes occurred in China, and the differences between the public health systems in the United States and that in China.

Given ongoing demographic shifts, family changes, and profound socioeconomic transformations in China, the rise of institutional elder care seems all but inevitable to meet the growing demand for long-term care of a rapidly aging population. As this new industry expands rapidly, it is imperative for Chinese policymakers to institute a formalized regulatory structure as soon as possible. To this end, building an information infrastructure is essential to enhance regulatory oversight and quality monitoring in Chinese long-term care facilities.

The ultimate goal of comparative effectiveness research (CER) is to develop and disseminate evidence-based information about which interventions are most effective for which patients under what circumstances. To achieve this goal it is crucial that researchers in methodology development find appropriate methods for detecting the presence and sources of heterogeneity in treatment effect (HTE). Comparing with the typically reported average treatment effect (ATE) in randomized controlled trials and non-experimental (i.e., observational) studies, identifying and reporting HTE better reflect the nature and purposes of CER. Methodologies of CER include meta-analysis, systematic review, design of experiments that encompasses HTE, and statistical correction of various types of estimation bias, which is the focus of this review.

Objective: To describe the documented evidence concerning agricultural injury in China and to identify topics for future research.
Method: Literature search and review were conducted to collect publications that were relevant to agricultural injury in China. The process included defining agricultural injury for the purpose of this study, selecting articles according to inclusion criteria and extracting data from each paper. Descriptive methods were used to analyze the contents, research approaches, distribution of authors, and cooperation percentage of agricultural injury studies.
Results: After applying the inclusion criteria, 89 articles were included in this study. The author collaboration percentage (number of articles with more than one author divided by number of total articles) and the institutional collaboration percentage (number of articles with more than one organization divided by number of total articles) among the 89 articles were 85.4% and 42.7%, respectively. Most of the authors are affiliated with a Center for Disease Control and Prevention (CDC) or an academic institution located in 10 of the 31 provinces in mainland China. Among the 89 articles, only 6 were on injuries related to agricultural work, the rest (83) dealt with injuries among rural residents with or without clarifying occupations or ongoing activities.
Conclusions: Research on agricultural injuries in China is currently in its early stage. More research is needed to obtain evidence that can be used in policy making for agricultural injury control. Our study is the first to describe the documented evidence on agricultural injuries in China and identify topics for future research.

Dr. REN is the Director-General of the Department of International Cooperation at the Ministry of Health (MOH) of the People’s Republic of China. Throughout his career, Dr. REN has been involved in numerous health policy research projects including the “Health care systems research in rural China” (MOH), “Financing and organization of health care services in poverty areas of China” (World Bank), “The pilot study of health insurance reform in urban China” (Chinese State Council) and the “Experiment in community health protection-policy exploration, training and demonstration program” (UNDP). In recent years, as a senior representative of the Chinese government, he served as a leading board member in a number of international organizations including WHO, UNAIDS, and the Global Fund to Fight AIDS, TB and Malaria. He is also the point of contact for many Chinese bilateral cooperation mechanisms in health. Dr. REN received his MD, MPH and PHD from Xi’an Medical University in 1987, Harvard School of Public Health in 1993 and Peking University in 2008, respectively.
Dr. Lingling Zhang initiated this interview when she met with Dr. REN at the Harvard America-China Health Summit organized by the Harvard School of Public Health China Initiative. Dr. REN was one of the invited guests of honor at the Summit. Dr. REN responded to Dr. Zhang’s questions in writing after he returned to China.

A Brief Introduction to Four National Health Databases in the United States

Health and medical care pervade every aspect of our lives. As the world population ages, an entirely new demographic stress will be put on healthcare systems. Some estimates project healthcare costs in the United States to account for 20% of GDP in the next few years. In comparison, China’s total expenditure on health as a percentage of GDP has risen steadily and it is currently below 6%. However, with the rise of life expectancy, an aging population and higher living standards, health expenditure in China will continue to rise. Another challenge China faces is healthcare system reform to ensure equity and to reduce health disparity. To solve these challenges, accurate collection of health statistics at the national level is needed.
In this article, we introduce and compare four national health databases in United States, which are pillars for evaluating national health profile and for formulating national health policies. The four databases are National Health Interview Survey (NHIS), Medical Expenditure Panel Survey (MEPS), National Health and Nutrition Examination Survey (NHANES) and Behavioral Risk Factor Surveillance System (BRFSS). As an example, we illustrate how to derive the prevalence of cholesterol screening from the four databases. Despite differences, the overall distributions follow similar patterns across four datasets. These databases can be linked with other data sources to answer more complicated questions in health and healthcare.
We hope that this article can draw the attention of Chinese health researchers and policymakers on the importance of health surveillance and can lead to more discussions and interest on how China can benefit from the U.S. experience in conducting health surveillance at the national level.

Dr. Teh-wei Hu is a Professor Emeritus of Health Economics and served as Associate Dean and Department Chair in the School of Public Health at the University of California, Berkeley. Currently he serves as the Director for International Tobacco Control Policy Research and Evaluation at the Public Health Institute. He has been appointed by the U.S. Government as a member of the Interagency Committee on Smoking and Health. Professor Hu’s areas of expertise are the application of econometrics to health care services research. He is the author of more than 200 publications. He contributed chapters in major WHO and World Bank tobacco control publications. During the past 20 years, he has been conducting research in economics of tobacco control in the US, China, Southeast Asia, and Estonia. He is the editor of Tobacco Control Policy Analysis in China: Economics and Health (2008). He co-authored a report on Tobacco Taxation and Its Potential Impact in China (Hu, Mao, Shi, and Chen, 2008) funded by the Bloomberg Philanthropies and the Gates Foundation. He has served as a consultant to the World Bank, the World Health Organization, and a senior policy advisor to the Ministry of Health, China.

This paper examines the adoption and use of “biodigesters”, a simple waste-to-energy toilet technology in China. Through interviews with stakeholders directly involved and impacted by water and sanitation development in southwestern China, this paper explores the incentives and barriers to scaling-up biodigesters in rural China. It further examines the extent to which biodigesters are in-line with China’s national and local public health priorities, as well opportunities to establish best practices in the emerging private and not-for-profit sectors. After four decades of use in China, the successes and shortcomings of biodigesters are known, and the experience offers valuable lessons to worldwide rural health development and sanitation campaigns.

Resolving and Preventing Medical Disputes in China

Medical disputes have been a major issue for the Chinese government. Though Chinese government implemented regulations on handling medical accidents in 2002, annual incidents of medical disputes have dramatically increased from 6,324 cases in 2003 to 115,000 cases in 2014. Several factors led to the medical disputes from the perspectives of patient satisfaction, physician performance and the healthcare system. This paper explores the reasons underlying the complex issue and tangible solutions to address the medical disputes in the future.
医疗纠纷已成为困扰中国的一件大事。尽管中国政府于2002年推行了《医疗事故处理条例(草案)》, 医疗纠纷事件从2003年的6324起急剧上升到2014年的11,5000起。从病人满意度,医生行为和医疗体系等方面看,医疗纠纷并非单一因素造成的。此文剖析了医疗纠纷的成因,并提供了未来解决医疗纠纷的可行方法。

Professor BA Denian is an elected member of the Chinese Academy of Engineering and the Institute of Medicine of the U.S. National Academies. He has dedicated his academic career to tumor immunology research. In the 1980s, he discovered a natural thymocytotoxic autoantibody, and proposed a theory to illustrate the relationship between abnormal immune function and hypertension, and thus is recognized as a pioneer of cancer biology and theory in China. Dr. Ba had practiced medicine for many years and trained numerous medical professionals. He served as the president for both Peking Union Medical University and the Chinese Academy of Medical Sciences for nine years. Dr. Ba served as the Dean of Zhejiang University School of Medicine from 2003 to 2009. Although Dr. Ba has retired from these positions, he remains active and at the forefront of health research in China.

Tide Turning: China’s Health Care Policy in Transition

This article systematically reviews the historical process of China’s healthcare system reform, and provides his prospective on the future reform direction. The author examined the unintended consequences of China’s market-oriented healthcare reform since the collapse of the socialized health system in 1979, including the sharply decreasing health insurance coverage, diminishing public health prevention services, increasing healthcare costs and worsening inequalities, resulted from the reduction in public health expenditures. In response to the collapse of universal health system, a new Urban Basic Medical Care Insurance System for Staff and Workers (UBMCI-SW) was started in 1993 for urban employees; the New Rural Cooperative Medical System (NCMS) was reestablished in October 2002 for the rural residents; and various forms of medical care insurance schemes were established starting in 2007 to provide coverage for the elderly, children and migrant farmer labors. China has achieved a significant success in restoring its universal healthcare system in one decade, in spite of the limited health services and scope of illness coverage. Witnessing the currently heated debate over whether the future reform direction should focus on marketization or public welfare, Dr. Guo believes that the reform-minded China’s new government will increase public expenditure and focus more on the welfare function of healthcare system.

Dr. Richard Rothenberg joined the Institute of Public Health, Georgia State University as a Professor in May 2007. He had previously worked at Emory University as Professor of Medicine (1993-2007) and prior to that had spent 25 years at the Centers for Disease Control and Prevention (CDC). Over the past several decades, he has conducted research that examines the dynamics of transmission of infectious diseases, primarily HIV, STDs, and the blood borne illnesses (BBIs), with particular emphasis of the effects of social, sexual, and drug-using networks on transmission. He is currently P.I. on an NIH/ NIDA-sponsored project focused on the geographic aspects of social network interactions. Dr. Rothenberg has published over 150 peer-reviewed articles, primarily in the fields of STD, HIV, and epidemiologic methods. In 2002, he received the Parran Award for lifetime contributions to the field of STDs. He is the current Editor-in-Chief of the Annals of Epidemiology. This interview was conducted by Dr. Zheng (Jane) Li with Dr. Rothenberg on November 9, 2012.

Job Satisfaction by Chinese Primary Care Doctors Following Health Care Reform

The purpose of this study is to compare primary care doctors’ job satisfaction and factors associated with it before and after the latest health care reform in China. Data for the study were obtained from China Primary Care Workforce Surveys conducted in 2008 and 2011. Compared to results from the 2008 survey, primary care doctors (PCDs) in the 2011 survey were more satisfied with their jobs overall as well as work conditions and equipment, but less satisfied with their income. In both surveys rural CHC and village clinic doctors were less satisfied than their urban counterparts with their jobs overall, income, work condition, and equipment. Logistic regressions showed that practice setting (i.e. urban, rural, or village) and educational level were two important factors associated with job satisfaction. These findings demonstrated both significant achievements and further efforts to be made to strengthen primary care workforce and enhance their job satisfaction.

Chou, Chiu-fang
Dr. WANG Yu graduated from Beijing Medical University in 1982. He continued onto his graduate study at the Institute of Hepatology, Beijing Medical University from 1983 to 1989 and received his Master’s and Doctorate of Medicine during this period. From 1991 to 1993, he studied at the Jichi Medical School in Japan and received a Ph.D. degree in Preventive Medicine. In 1999, He completed Master of Business Economics program from the Chinese Academy of Social Sciences. Starting in 1985, he had been an assistant professor, associate professor, professor, deputy director and director of the Institute of Hepatology in Beijing Medical University, specializing on hepatology and viral molecular biology. Dr. Wang had participated in and chaired a number of national scientific and technological projects and programs, such as the Natural Science Foundation of China. He served as a committee member of the Chinese Medical Association and vice chairman of the Medical Virology Branch of the Chinese Medical Association. Since 1996, he had served as the Executive Vice-Director and Director of the Office of Scientific Research in Beijing Medical University, and Deputy Dean, School of Medicine, Peking University. From 2000 to 2003, he was appointed Deputy Director of the Center for Biological Engineering and Development, China Ministry of Science and Technology. From 2003 to 2004, he was Deputy Director of the Office of Rural and Social Development at the Ministry of Science and Technology. In June 2004, he was appointed as the Director of Chinese Center for Disease Control and Prevention under the Ministry of Health, China.
Dr. Wang was invited to have a phone interview with the China Health Policy and Management Society in August. The interview was conducted by Dr. Chiu-fang Chou on September 8, 2012. Drs. Zhuo (Adam) Chen and Zheng Li from CHPAMS also participated in the interview. CHPAMS acknowledges help from Ms. Doris Wang, Mr. Lin Wang, and Ms. Xuhong Ding in facilitating the interview.

As a newly emerging pattern of consumption and production of healthcare services, medical tourism is featured with particular regional characteristics in the range of service for patients elect to travel across international borders with the intention of receiving medical treatment. From perspectives of globalization and supply consideration, this article illustrates the drives, size and scope of medical tourism. Experiences from Korea and UK are introduced to explain commonalities and distinctions of diverse medical tourism treatments and their destinations. Professor Hengjin Dong joined the Center for Health Policy and Management Studies, Zhejiang University School of Medicine in July 2009. Prior to that, he was the lead of Junior Group of International Health Economics and Health Financing at Institute of Public Health, Heidelberg University since 2006. He was a Senior Research Fellow at Brunel University (2004-2006) and Heidelberg University (2000-2004). He also had served 14 years at Fudan University Shanghai School of Public Health (formerly Shanghai Medical University) before he moved to Europe. His research focuses on health policy, health economics, healthcare financing, pharmaco-economics and health technology assessment. He has been an associate editor of BMC Health Services Research since 2012 and he is a member of the editorial board of Pharmacoeconomics. Ms. Yan Ding (PhD candidate) interviewed Professor Dong on April 24, 2013.

The impact of China’s ‘Zero-markup Policy for Essential Drugs’ on patients, county hospital revenue and government subsidy levels

经倍差法分析,药品零差率政策对病人的就医费用和医院收入有显著影响。政策实施后,次均门诊费用降低19元,次均住院费用降低近400元;县级医院的医疗服务提供量增加、医院总收入增加的同时,药品收入减少。从政府补贴的角度看,我们的研究测算了确保医院收入年增长率5%的情况下,药品零差率政策实施时政府应该补贴给医院的额度。分析表明,即使药品的价格下降,因为医疗服务的使用率增加,医院收入增加,政府补贴的负担很小。经系统分析,药品零差率政策可以在零补贴或者是少量补贴的情况下可持续地实施。
In 2009, China implemented a Zero-markup Policy for Essential Drugs (ZPED), a policy with potential to dramatically improve health in the country. This series of studies aims to evaluate the impact of ZPED on patients, county hospital revenue and government subsidy levels.
Data from Ningshan and Zhenping county hospitals were collected. The hospitals were selected because they were comparable in many ways, with the exception of Ningshan having implemented ZPED during a period when Zhenping had not. Ningshan, therefore, was used as the experimental group and Zhenping the control group. To compare outcomes of interest between the counties, the primary method of analysis was a difference-in-differences model.
The results showed that ZPED had an effect on patients’ medical expenditure, county hospital revenue and government subsidy levels. With regard to patients, in absolute terms, the total expense per visit reduced by 19.02 CNY (3.12 USD) for outpatient services and 399.6 CNY (65.60 USD) for inpatient services. In relative terms, the expense per visit was reduced by 11% for both outpatient and inpatient services. With regard to the impact on county hospital revenue, for Ningshan County hospital, ZPED led to an increase in health care provision and a sustained total hospital income despite a decrease in drug revenue. Lastly, with regard to government subsidy levels, the research demonstrates that with minimal or no subsidy, the government can catalyze the zero-markup policy and potentially generate positive outcomes for county hospitals. Overall, this research showed that ZPED has led to positive outcomes for patients, county hospitals and the government.

Dr. YU Hai is a Professor in Zhejiang University School of Medicine. He also serves as the Director of International Education Program and the Director of Center of Distance Education in Zhejiang University School of Medicine. Dr. Yu studied at Shanghai First Medical College (now Fudan University Shanghai Medical College) from 1962 to 1968. During 1968-1978, Dr. Yu served as a physician in Longde County in the Ningxia Hui Autonomous Region. He was enrolled in the graduate program in Ningxia Medical College in 1978 and then studied in Leeds University in U.K. during 1980-1983. Dr. Yu received his PhD degree in Cancer Research from Leeds University and returned to China in 1983. He has served as a Professor, Deputy Director of the Cancer Institute, Zhejiang Medical University, Vice President of Zhejiang Medical University, and Associate Dean of Zhejiang University School of Medicine. His main areas of interest are cancer immunology, cancer biotherapy, and cancer prevention studies. Dr. Yu has authored and co-authored over 100 publications. He has won more than 10 times the Award for Science and Technology Progress at the national and provincial levels.
余海教授简介:余海教授现任浙江大学教授,浙江大学医学院国际教育项目主任,医学远程教育中心主任。他1962至1968年毕业于上海第一医学院医学系,1968年至1978年任宁夏回族自治区隆德县医生。1978年余教授考取宁夏医学院研究生并于1980年留学英国,1983年取得英国LEEDS大学肿瘤研究所博士学位,同年学成归国。余教授历任浙江医科大学教授,浙医大肿瘤研究所副所长、浙江医科大学副校长、浙江大学医学院副院长。他主要从事肿瘤免疫学、肿瘤生物治疗和肿瘤预防学研究,在国内外发表论文100多篇,曾10多次获国家级和省级科技进步奖。

UNDERSTANDING INFANT AND YOUNG CHILD FEEDING CHALLENGES IN CHINA

This paper reviews infant feeding challenges China faces in the current economic and social climate. Infant and young childhood is a critical period of growth and development and losses due to under-nutrition are often irreversible. In urban areas, there has been a rapid increase in childhood obesity since the market reform policies of the early 1980’s, with interventions focusing on school-aged children or young adults. Under-nutrition continues to be widespread in many rural areas of China, and while improvements have taken place, most efforts are focused on school-aged children. In both under- and over-nutrition, little attention has been paid to the role infant feeding plays. Through observations and interviews with healthcare workers, mother’s groups and rural-urban migrant women in Shanghai and Yunnan, we attempt to deconstruct social and economic determinants of infant and young child feeding practices in order to illuminate specific barriers and possible solutions. Infant feeding decisions, particularly those regarding breastfeeding, are closely linked to cultural, economic and social values. Education, a crucial component of improving nutritional outcomes, does not alone change infant feeding behavior. Rural-to-urban migration, re-negotiation of family roles, and media as the main source of nutrition information for households each pose unique barriers to providing infant and young children with proper nutrition. Infant feeding and nutrition programs should take a multi-pronged approach that includes education, awareness, and policy.
摘要: 本文讨论在当前经济和社会环境下,中国所面临的婴儿喂养问题。婴儿和幼儿期是生长发育的关键期,在此阶段由于营养不足而造成的不利影响常常是无法弥补的。在中国很多农村地区,营养不足仍然相当普遍,虽然相应的改进措施已经实施,但是大多数只关注于学龄期儿童。在城市中,自80年代市场改革开放以来,儿童肥胖迅速增长,而相应的干预措施也只关注于学龄期儿童及青少年。无论是营养不足还是营养过剩,都没有注意到婴儿喂养在其中所起的作用。教育,虽然是改善营养状况的关键因素,但是单靠教育难以改变婴儿喂养习惯。从农村到城市的移居,家庭成员角色的重新定位,以及家庭营养信息主要来自于媒体等,各自从不同方面对婴幼儿获得适宜营养发挥着独特的作用。因此,婴儿喂养和营养计划应该多管齐下,包括教育、宣传、政策制定等。
